After Transferring Paperwork
When you schedule a new patient appointment, we will have you sign a release of records from your previous doctor so we can obtain and review your records prior to your appointment. If you have a copy of your child's immunization records, please bring them to the first visit.

To do this successfully, we must be able to review all of your medical records from all providers that you have seen. It is our policy to receive your previous medical records before we can assume care and schedule an appointment.
